Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year experience in home care.
• Knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic technology skills including operation of a mobile device.

Responsabilités

• Represent the branch in external transitional care activities and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible for education related to provided services.

Formation

Graduate of an approved school of clinical education

Licensed in the state where they currently practice

At least one year experience in home care

Knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations

Basic technology skills including operation of a mobile device
