Job summary

Interim HealthCare Inc. seeks a Hospice Care Transition RN (CTN) to coordinate and process patient transitions across the care continuum. You will ensure timely assessments, provide care information to families, and support hospital staff with post-acute referrals.

Flexible scheduling and full-time roles are offered, with daily pay options and 1:1 patient care. The position emphasizes patient-centred transitions and quality improvement across services.

Qualifications

• Current unrestricted RN license required.
• At least 1 year of professional RN experience.
• Reliable transportation to care sites/work locations.
• Previous home care or hospice experience preferred.
• Sales or marketing experience preferred.

Responsibilities

• Facilitate and coordinate transitional care to ensure timely discharge planning.
• Provide families and patients with information to manage care post-discharge.
• Support hospital staff during transition periods.
• Expedite referrals for post-acute care (home care, hospice, rehab, SNF).
• Improve transition to home care/hospice teams and share post-acute insights.

As a Hospice Care Transition Registered Nurse (CTN), you will:

• Ensure timely and responsive care by providing daily onsite access to transitional care coordination and ensuring timely assessment of patient and family needs prior to discharge
• Provide information to families and patients to manage care
• Support the hospital staff
• Expedite referrals for any type of post-acute care (home care, hospice, palliative, personal care and support services, skilled nursing facilities & rehab)
• Improve the transition to the home care/hospice teams
• Provide insights on post-acute care trends

To qualify as a Hospice Care Transition Registered Nurse (CTN) with us, you will need:

• Licensure: Current unrestricted license to practice as a Registered Nurse (RN) required
• One (1) year of professional experience practicing as listed above, required
• Reliable transportation to/from care sites and/or work locations
• Previous experience in home care or hospice preferred
• Previous experience in sales/marketing preferred
