Interim HealthCare Inc. is seeking a Hospice Care Transition Registered Nurse (CTN) to facilitate patient transitions across the continuum of care. You will coordinate transitional care, communicate care plans to families, and support hospital staff during discharge and post-acute referrals.

Qualifications

• Current unrestricted RN license required.
• Minimum 1 year of professional nursing experience.
• Reliable transportation to care sites/work locations.
• Home care or hospice experience preferred.
• Sales/marketing experience preferred.

Responsibilities

• Provide timely, on-site transitional care coordination prior to discharge.
• Inform patients and families to manage care after discharge.
• Support hospital staff during transitions.
• Expedite referrals for post-acute care (home care, hospice, SNFs, rehab).
• Improve transitions to home care/hospice teams.
• Provide insights on post-acute care trends.
