Qualifications

• RN license in good standing is required.
• Minimum 1 year of professional experience in hospice, home care, or CTN roles preferred.
• Reliable transportation to/from care sites and/or work locations.

• Provide daily on-site access to transitional care coordination and timely assessments prior to discharge.
• Inform patients and families to manage care effectively.
• Support hospital staff in transition planning and care delivery.
