Job summary

Interim HealthCare in Dayton, OH seeks a Hospice Care Transition Registered Nurse (CTN) to coordinate and facilitate post-acute care transitions with a patient-centered approach.

You will provide timely on-site care coordination, educate families, support hospital staff, and streamline referrals to home care, hospice, or SNF settings. Emphasis on quality, responsiveness, and compassionate support.

Qualifications

• RN license required and unrestricted.
• Minimum 1 year professional RN experience.
• Reliable transportation to care sites or work locations.
• Home care or hospice experience preferred.
• Sales/marketing experience preferred.

Responsibilities

• Provide daily onsite transitional care coordination and timely assessments before discharge.
• Inform patients and families to manage care.
• Support hospital staff.
• Expedite referrals for post-acute care (home care, hospice, palliative, SNFs & rehab).
• Improve transitions to home care/hospice teams.
• Provide insights on post-acute care trends.

As a Hospice Care Transition Registered Nurse (CTN), you will:

• Ensure timely and responsive care by providing daily onsite access to transitional care coordination and ensuring timely assessment of patient and family needs prior to discharge
• Provide information to families and patients to manage care
• Support the hospital staff
• Expedite referrals for any type of post-acute care (home care, hospice, palliative, personal care and support services, skilled nursing facilities & rehab)
• Improve the transition to the home care/hospice teams
• Provide insights on post-acute care trends

To qualify as a Hospice Care Transition Registered Nurse (CTN) with us, you will need:

• Licensure: Current unrestricted license to practice as a Registered Nurse (RN) required
• One (1) year of professional experience practicing as listed above, required
• Reliable transportation to/from care sites and/or work locations
• Previous experience in home care or hospice preferred
• Previous experience in sales/marketing preferred
