Job summary

Jobot is seeking a Hospital Clinical Care RN for a full-time, onsite role in Manhattan. The RN will coordinate hospice care between hospital staff, patients, and families, and work with hospice referral sources across hospital and outpatient settings.

The position involves coordinating admissions, assessing patient cases, and collaborating with hospice physicians and facility teams to ensure appropriate care. A strong focus on patient and family education supports improved outcomes.

Qualifications

• RNs prepared to work with Hospice referral sources in hospital and outpatient settings.
• Coordinate admissions and care transitions with hospice physicians and clinical teams.
• Provide patient and family education to support hospice care.

Responsibilities

• Serve as intermediary between Hospice, hospital staff, patients and families to coordinate care.
• Perform chart assessments and bedside evaluations to determine hospice eligibility.
• Collaborate with facility teams to support patient care decisions.
• Facilitate hospice admissions and seamless transitions for patients and families.
• Assist with disposition planning and coordinating equipment needs.
