Job summary

Northwell Health is seeking a Hospice Care Registered Nurse for Staten Island, NY. This per diem role focuses on compassionate, comprehensive nursing care in a hospice setting, using the nursing process to assess, plan, implement, and evaluate patient care.

The ideal candidate will collaborate with the health care team, educate patients and families, administer medications, and ensure safety while tailoring plans to individual needs. NY RN license and BCLS are required; ACLS preferred.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Must be licensed to practice as a Registered Professional Nurse in New York State.
• BCLS certification required; ACLS preferred.
• Per diem schedule availability may be required.

Responsibilities

• Assess patient physiologic health via interviews and chart reviews.
• Evaluate gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
• Assess mobility, sensory deficits, and skin condition; adjust care plans as needed.
• Assess psychosocial health status and identify cultural/ethnic requirements.
• Develop criteria for measuring patient outcomes and establish individualized goals.
• Formulate and document the plan of care, coordinating with health team members.
• Implement the plan of care, ensuring safety precautions are followed.
• Administer medications correctly and educate patients on drug interactions.
• Participate in patient and family education, providing explanations of conditions.
• Collaborate with care team members to plan and carry out treatment regimens.

Experience

• Must be enrolled in an accredited program within 24 months of employment if hired after September 1, 2010, and obtain a BSN degree within five (5) years of employment date.

Licenses & Certifications

• Current license to practice as a Registered Professional Nurse in New York State.
• BCLS certification required; ACLS preferred.
