Job summary

Gentiva Hospice is seeking a Hospice Care Consultant (HCC) to drive strategic referral development and educate healthcare partners about hospice services. You will cultivate relationships with hospitals, SNFs, home health agencies, clinics, and physician offices, while promoting Gentiva’s mission and improving patient access to care.

The role emphasizes consultative marketing, independent work, and collaboration with clinical and operations teams to deliver compassionate, high-quality hospice

Qualifications

• Bachelor's degree or equivalent in a related field.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong relationships with physicians, hospitals, SNFs, and home health professionals.

Responsibilities

• Educate referral sources to ensure appropriate patient referrals.
• Identify new referral opportunities by visiting hospitals, SNFs, home health agencies, clinics, and physician offices.
• Engage healthcare professionals in discussions about patient needs and hospice options.
• Plan and execute marketing initiatives to raise awareness and drive referrals.
• Represent Gentiva Hospice at events and professional trainings to promote services.
• Advise patients and families about hospice options and benefits of early referrals.
