Job summary

Affinity Hospice is seeking a qualified Registered Nurse for full-time case management and clinical leadership in the Madison, AL area. You will coordinate interdisciplinary care, develop patient plans, and ensure high-quality hospice services in patients' homes and communities.

The role requires an RN license, CPR certification, and at least two years of RN experience in hospice, home health, or clinical settings. Driving to patient homes and strong communication are essential.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse (RN).
• Current CPR certification.
• Minimum of two years of RN experience in clinical, home health or hospice settings preferred.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care with infection control measures protecting staff and patient.
• Ensure continuity of quality patient care with proper documentation.
• Supervise hospice aides and licensed practical nurses.
