Job summary

Affinity Hospice in Birmingham, AL is seeking an RN Case Manager for a full‑time, 40‑hour schedule. The role focuses on planning and directing hospice care, utilizing community resources and collaborating with families to meet patient needs at home and in community settings.

The ideal candidate is a registered nurse with CPR, a valid Alabama license, and strong communication and clinical skills. The position offers a comprehensive benefits package and opportunities for professional growth.

Qualifications

• Graduate of accredited Diploma/Associate/Bachelor Nursing programs.
• Current RN license in the state where practicing.
• Current CPR certification.
• Two years RN experience in clinical care, home health or hospice preferred.
• Must have a valid driver’s license and insured vehicle.
• Ability to lift and transfer patients and supplies.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop and document the patient plan of care.
• Provide care using infection control measures to protect staff and patients.
• Ensure quality patient care with proper documentation.
• Monitor cases for third‑party payer requirements.
• Support agency policies and participate in on‑call duties.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned.
