Job summary

Hospice of North Alabama in Huntsville, AL is seeking a Registered Nurse Case Manager to join our hospice team. This full-time role involves direct patient care, care planning, and collaboration with an interdisciplinary team to support patients and families.

Qualified candidates are RN-licensed, with at least 1 year of nursing experience (3+ years preferred); hospice experience is a plus. A valid driver’s license and insured vehicle are required.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in the state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients' wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management; educate and support families.
• Collaborate with interdisciplinary team; maintain accurate documentation; participate in on-call rotation.
