Job summary

Queen City Hospice – an Addus family company seeks a Hospice Transitional Care Navigator in Mason, OH. You will develop upstream referral relationships with physicians and hospitals, acting as a clinical consultant and educator to care teams, and identifying patient needs and hospice suitability.

This role requires strong communication, strategic thinking, and a hunter-sales mindset. You will work in-market, coordinating with hospital staff and field sales to drive appropriate hospice referrals

Qualifications

• Clinical-sales experience in healthcare or hospice settings.
• Ability to build and leverage upstream referral relationships.
• Strong communication and stakeholder management skills.

Responsibilities

• Build upstream-referral relationships with physicians and hospitals.
• Serve as hospice clinical consultant and educator to care teams.
• Review hospital records and engage patients/providers for hospice suitability.
• Identify and contact attending/consulting providers to educate on hospice.
• Collaborate with field sales to schedule consultative meetings.
• Maintain care-team workflows for documentation and education planning.
• Identify trends and partnership opportunities for earlier hospice engagement.
• Foster trusted external-care-team relationships to improve outcomes.
• Communicate with hospital leadership and care coordination teams.
• Meet productivity targets including timely medical-record reviews.
