Addus HomeCare in Oak Lawn, IL is seeking a Hospice Transitional Care Navigator to expand upstream hospice referrals and educate physicians, hospitals, and providers.

This field-based, non-virtual role requires clinical sales experience, hunter mindset, and strong communication; you will build relationships, assess patient needs, coordinate care teams, and promote dignified end-of-life hospice services.

Qualifications

• Clinically trained and experienced in business development and sales.
• Self-driven with hunter-sales mentality and curiosity.
• Strong communication, strategic thinking and problem-solving abilities.
• Bachelor's degree in marketing, business administration or related field; RN preferred.

Responsibilities

• Build upstream-referral relationships with physicians, hospitals and providers.
• Serve as hospice clinical consultant and educator to care teams.
• Review hospital records, identify decline patterns and eligibility.
• Educate on hospice appropriateness and discharge planning.
• Coordinate with field sales to schedule consultative meetings.
• Maintain workflows for patient information, education and meetings.
• Meet productivity targets including timely medical record reviews.
