Addus Home Care / JourneyCare Hospice is seeking a Hospice Transitional Care Navigator to expand upstream hospice referrals in Oak Lawn and surrounding markets. The role focuses on building physician and hospital partnerships, educating care teams, and identifying patients who would benefit from hospice services.

The ideal candidate has clinical background with sales/relation-building skills, a hunter mentality, and strong communication.

Qualifications

• Bachelor’s degree in marketing, business administration, or related field preferred.
• Minimum of two years relevant experience in healthcare, hospice, or sales.
• RN license in state of practice preferred.

Responsibilities

• Build upstream-referral relationships with physicians, hospitals, and providers.
• Serve as hospice clinical consultant and educator to hospital care teams.
• Review hospital documentation and identify decline patterns to support admissions.
• Educate on hospice appropriateness and discharge education for referred patients.
• Identify attending/consulting providers to facilitate patient assessments and education.
• Collaborate with field sales to schedule consultative meetings with external care teams.
• Maintain workflows for patient information, education delivery, and meetings.
