Job summary

Chapters Health System seeks an RN, Care Coordinator to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group, and provide palliative and supportive care in the patient/family unit.

The position requires an active RN license and strong communication with the IDT. Duties include home visits, education, EMR documentation, and collaboration with physicians to reflect evolving care needs.

Qualifications

• Current RN license in the state of employment.
• Minimum one year nursing experience; hospice or hospital preferred; Florida Alzheimer's Training per policy for PACE.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance; reliable transportation.
• Ability to visit participants at home; English communication skills.
• Ability to perform physical duties and the essential functions; active BLS.

Responsibilities

• Provides and manages direct care to patients and families as part of IDT, with psychosocial and physical components.
• Coordinates the Plan of Care reflecting evolving patient needs.
• Educates patient and family about disease, palliative interventions, safety and caregiving.
• Conducts home visits to assess safety, medication and nutritional compliance.
• Documents in EMR; communicates with IDT; initiates care plans.
• Participates in IDT meetings; communicates with physicians and staff; may supervise CHWs.
• During emergencies, may report to a designated location to ensure continuity of services.
• Performs other duties as assigned.
