Qualifications

• Graduate of an approved school of professional nursing and current, unrestricted RN license in Utah.
• One year of nursing experience preferred in hospice, home health, oncology, geriatrics, or end-of-life care.
• Knowledge of hospice philosophy, symptom management, care planning, and interdisciplinary care coordination.

Responsibilities

• Provides services in accordance with the plan of care and initial evaluation visit.
• Regularly reevaluates patient nursing needs and revises the care plan.
• Completes and documents OASIS assessments and progress notes in a timely manner.
