Job summary

Home Caregivers Partnership is seeking a Hospice Case Manager RN to coordinate and provide comprehensive nursing care for hospice patients and families in Ogden, Utah. You will assess needs, develop care plans, and ensure accurate clinical documentation under the supervision of the Clinical Manager.

The ideal candidate holds an approved nursing degree with an active Utah RN license and a commitment to hospice philosophy, with strong assessment and communication skills and reliable transportation.

Qualifications

• Graduate of an approved school of professional nursing and current, unrestricted RN license in Utah.
• One year nursing experience preferred (hospice/home health/geriatrics/oncology/end-of-life care).
• Knowledge of hospice philosophy, symptom management, care planning, and interdisciplinary care coordination preferred.
• Valid driver's license, reliable transportation, and required automobile insurance.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and develops the care plan in coordination with the IDT.
• Regularly reevaluates patient nursing needs and revises the care plan as needed.
• Furnishes services requiring specialized nursing skills and initiates preventive/rehabilitative procedures.
• Prepares clinical notes and summarizes care conferences in a timely manner.
• Coordinates services with IDT members, physicians, and caregivers as appropriate.
• Informs physician of changes in patient condition and needs; counsels patient and family.
• Supervises LPN/LVN and HA/CNA per regulatory requirements; assigns home health aides.
• Performs OASIS assessments and other required documentation per agency policy.
• Maintains confidentiality and adheres to agency policies; attends staff meetings and in-service programs.
