Responsibilities

• Coordinate the overall care of the patient with family and health professionals.
• Provide skilled home health and hospice nursing in the patient’s home.
• Perform initial and ongoing assessments to determine needs.
• Conduct visits for pain management, wound care and meds as ordered.
• Complete documentation accurately and timely.
• Monitor and supervise team members involved in patient care.
