Job summary

Hospice de la Luz seeks a Clinical Navigator (RN/LPN/LVN) to join its business development team. You will qualify referral accounts, educate providers about hospice and palliative care, and coordinate timely transitions from hospital or clinic to hospice.

This is a field-based, clinician-led sales role with weekly referral-growth expectations. You will build relationships with physicians, nurses, discharge planners, and office staff, ensuring compliant, patient-centered transitions while

Qualifications

• Current, unrestricted RN or LPN/LVN license in the state or multistate compact.
• Minimum 2 years of clinical nursing experience, preferably in hospice, palliative care, home health, oncology, cardiology, pulmonology, or acute care.

Responsibilities

• Develop and manage a territory of referral accounts including hospitals and specialty practices.
• Educate providers on hospice and palliative care eligibility and Medicare benefits.
• Coordinate smooth transitions of care from hospital/clinic to hospice or palliative care.
• Meet weekly visit and referral-growth targets and document activity in the CRM within 24 hours.
