Hospice de la Luz seeks a Clinical Navigator (RN/LPN/LVN) to join the field-based clinical development team in Albuquerque, NM. The role blends bedside nursing credibility with a physician and hospital referral focus, educating providers, and coordinating timely transitions to hospice or palliative care.

You will identify target accounts, build relationships with physicians, nurses, and care managers, track referrals, and ensure compliant, compassionate care transitions while meeting weekly

Formación

• Licensed RN or LPN/LVN with unrestricted license in the required state or multi-state compact.
• Minimum 2 years nursing clinical experience, preferably hospice/palliative/home health.
• Ability to educate providers and staff on hospice and Medicare basics.
• Strong presentation and communication skills with physicians and clinical staff.

Responsabilidades

• Develop and manage a territory of accounts including hospitals and specialty practices.
• Build relationships with physicians, nurse practitioners, case managers, discharge planners and practice managers.
• Educate providers on hospice eligibility, benefits, and disease indicators.
• Coordinate transitions of care from hospital/clinic to hospice or palliative care.
• Track referral trends and market changes; share insights with leadership.
• Ensure all activities comply with HIPAA and CMS guidelines.
