Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Previous Hospice or Home Health experience required.
• 2+ years of leadership or care coordination experience preferred.
• Proven ability to work within an interdisciplinary team.

Responsibilities

• Coordinate hospice patient care across the interdisciplinary team.
• Manage physician orders and ensure timely entry.
• Support timely admissions and follow up on pending referrals.
• Lead development, implementation, and updates of care plans.
• Ensure compliance with hospice regulations and quality expectations.
