Qualifications

• High School Diploma preferred, plus training/certification in related field required.
• Certified Medical Assistant (CMA) is required and must be in good standing with the AAMA.
• Minimum 1 year experience required. Experience in homecare, geriatrics preferred.

Responsibilities

• Initiates contacts with patients and families to introduce services and answer questions.
• Schedules NP visits and manages appointment reminders and scheduling within geographic distance.
• Prepares weekly NP schedules and reviews clinician schedules to optimize capacity and productivity.
• Ensures clinicians are assigned efficiently to maximize care and capacity; follows up on missed visits.
• Handles calls from patients and staff regarding scheduling; refers issues to Manager of Practice Operations.
• Monitors hospitalized patients and ensures follow-up as needed.
• Maintains medical records and processes attachments per guidelines.
• Supports branch operations and coordinates with patients, clinicians, referral sources and staff.
• Attends interdisciplinary group conferences and maintains professional relationships.
• Documents accurately and timely per VIA Health Partners standards and licensure requirements.
• Maintains licensure/certification, CPR, CEU requirements and professional development.
