Job summary

Compassus in Savannah, GA is seeking a Hospice Team Coordinator to support the delivery of compassionate hospice care. You will coordinate patient data, admissions, and interdisciplinary team communications, ensuring accurate records and regulatory compliance.

You will work closely with clinicians and families to promote the Compassus philosophy, uphold ethical standards, and maintain efficient operations and documentation across the EMR and HCHB systems.

Qualifications

• Must have strong data entry and medical records management skills.
• Excellent written and verbal communication.
• Ability to maintain patient confidentiality.
• English language literacy essential.

Responsibilities

• Enter and manage patient data including referrals and admissions in the EMR/HCHB systems.
• Ensure admission documents are complete and signed by physician.
• Maintain accurate patient records and generate regulatory reports.
• Coordinate interdisciplinary team communication and caregiver notifications.
• Serve as central contact for hospice team and patients' families.
• Assist with IDT pre/post activities and care plans.
