Job summary

Providence at Home with Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care, coordinate patient records, and support the interdisciplinary team. Responsibilities include managing intake data, admissions, billing alignment, and communications with families.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.

Responsibilities

• Enter and manage patient data including referral and demographic updates.
• Ensure admission documentation is complete with physician CTI and order to admit.
• Upload documents into HCHB and EMR systems for accurate records.
• Monitor DME and supply orders and inform management of trends.
• Ensure timely processing of physician orders and billing alignment.
• Verify and upload paper visit records into EMR and HCHB.
• Organize documents in patient charts for easy retrieval.
• Coordinate with IDT and act as main contact for patients and families.
• Handle incoming calls and manage the phone system to ensure responsiveness.
• Assist the DCS with pre/post-IDT tasks and update plans of care.
