Job summary

Life Care Home Health is seeking a Hospice Patient Care Assistant to join a dedicated interdisciplinary team. The role supports clinical record management, patient and family communications, and documentation across the hospice program.

The position emphasizes timely care coordination, field assignments, and collaboration with physicians, nurses, and therapists to ensure compassionate, quality patient support.

Qualifications

• LPN or MA certificate and a state nursing license in good standing.
• Minimum 2 years of recent experience in acute/sub-acute care.
• Ability to perform nursing activities within LPN/CNA scope and support patients and families.

Responsibilities

• Open, maintain, and close clinical records according to policy; complete 7-day audits.
• Act as liaison between staff, volunteers, patients and families.
• Enter data into Mumms software and document visits.
• Coordinate care plans in LTC facilities; prepare binders for charts.
• Assist with scheduling, staffing, and ensure timely documentation.
• Notify appropriate team members of patient discharge and close files.
• Accept field assignments and on-call duties as needed.
• Order and monitor supplies and durable medical equipment.
