Job summary

Life Care Home Health is seeking a Hospice Patient Care Assistant to join our interdisciplinary team. You will help manage patient records, coordinate care and support families under the Director of Patient Care Services.

Responsibilities include maintaining accurate clinical records, liaising between staff and families, preparing care plan documentation, and assisting with scheduling and staff deployment. LPN or MA certificate with active license preferred; 2 years in acute/sub-acute care

Qualifications

• LPN or MA graduation certificate with active state license.
• Minimum 2 years of recent experience in acute/sub-acute care.
• Excellent communication and teamwork in hospice care.

Responsibilities

• Maintain accurate clinical records in approved format and perform 7-day audits.
• Act as liaison between staff, volunteers, patients and families.
• Prepare care plan documentation and materials for interdisciplinary team meetings.
• Assist scheduling and staffing and ensure timely documentation.
