Compassus is seeking a Hospice Team Coordinator to oversee delivery of hospice care and coordinate the interdisciplinary team to ensure compassionate, compliant patient care.

The role emphasizes accurate data handling, documentation, and collaboration with physicians, nurses, and families to support patient comfort and quality of life.

Formación

• High school diploma or GED required.
• At least two years of medical records or related support experience preferred.
• Excellent written and verbal communication.

Responsabilidades

• Accurately enters and manages patient data, including referral intake information and new admissions.
• Ensures all admission documentation is complete, signed, dated, and includes CTI and admission order.
• Uploads documents into the HCHB system for seamless record-keeping.
• Uses HCHB to maintain and update patient records and generates reports for regulatory compliance.
• Monitors durable medical equipment orders and informs management of trends or issues.
• Monitors timely return of physician orders and processes them for efficient care.
• Verifies and uploads paper-based visit records into the EMR and HCHB for complete histories.
• Ensures organized document storage within patient charts for easy retrieval.
• Confirms care level matches billable level for accurate billing.
• Collects decertification paperwork and uploads it into patient records.
• Notifies providers of patient discharge via coordination notes.
• Initiates medical records requests as needed and processes provider notifications.
• Facilitates coordination among IDT members to ensure holistic care.
• Acts as central contact for hospice team, patients, and families.
• Handles incoming calls, messages, and transfers, maintaining a welcoming atmosphere.
• Manages phone system and second system when needed to ensure seamless communication.
• Assists the Hospice DCS with pre- and post-IDT activities and updating plans of care.
• Sends UPOCs to attending physicians by mail or fax.
• Prepares patient binders and ensures information is organized.
