Job summary

CHESAPEAKE HEALTH PARTNERS INC is seeking an Admission Registered Nurse to lead initial patient assessments and education for hospice services. You will establish plans of care with families and coordinate with the interdisciplinary team across home, facility, and hospital settings.

The role requires Maryland RN licensure and at least 2 years in med/surg or home health, with hospice experience preferred. A field-based schedule includes day-evening rotations and on-call duties.

Qualifications

• Two or more years of med/surg or home health experience required.
• Hospice experience preferred.
• Proficiency with standard computer software (Word/Outlook) expected.

Responsibilities

• Perform admission assessments and establish initial plan of care with patient and family.
• Conduct Skilled Nursing Visits (SNVs) in various settings to address concerns and document care.
• Perform death visits and coordinate bereavement support with the team.
• Communicate changes in patient status to the interdisciplinary team and physicians.
