Job summary

Transition Home Healthcare & Pathways Hospice in Houston seeks an experienced Hospice Business Development Specialist to grow referrals, partnering with physicians, hospitals, SNFs, ALFs, and senior communities.

You will leverage an existing referral network to drive census growth, manage relationships, and represent the hospice at events—while meeting monthly and quarterly growth targets.

Qualifications

• Excellent communication, negotiation and relationship-building skills.
• Proven ability to meet or exceed hospice sales goals.
• Established relationships with Houston-area healthcare providers.

Responsibilities

• Develop and implement strategic plans to generate new referrals and grow admissions in North Houston and surrounding areas.
• Build and maintain relationships with physicians, case managers, discharge planners, hospital systems, SNFs, ALFs, and senior communities.
• Maintain a regular schedule of face-to-face meetings, in-services, and presentations to referral sources.
• Serve as liaison between referral sources and hospice clinical teams to ensure seamless communication and service.
• Meet or exceed monthly and quarterly growth goals.
• Maintain documentation of all sales activities through EMR (HCHB) and weekly reports.
• Attend industry events and marketing events as a hospice representative.
