Transition Home Healthcare & Pathways Hospice is seeking an experienced Hospice Business Development Specialist for the Houston market. The role focuses on developing relationships with physicians, hospitals, SNFs and other referral partners to drive census growth.

Ideal candidates have an existing network in hospice or healthcare sales, with a proven track record of meeting sales goals, and are comfortable traveling within the North Houston area. Competitive pay and benefits are offered.

Qualifications

• Minimum of 3 years of home health/hospice sales experience.
• Established relationships with Houston-area physicians, case managers, discharge planners, hospitals, SNFs, ALFs.
• Strong communication, negotiation, and relationship-building skills.

Responsibilities

• Develop and implement strategic plans to generate new referrals and grow hospice admissions in North Houston and surrounding areas.
• Build and maintain relationships with physicians, case managers, discharge planners, hospital systems, SNFs, ALFs, and senior communities.
• Maintain a regular schedule of face-to-face meetings, in-services, and presentations to referral sources.
• Serve as the liaison between referral sources and hospice clinical teams for clear communication and service.
• Meet or exceed monthly and quarterly growth goals.
• Document all sales activities in the EMR (HCHB) and weekly reports.
• Attend industry events and marketing events as a hospice representative.
