Job summary

PruittHealth Hospice Blue Ridge is seeking an LPN for a Hospice Bridge Nurse role, providing care in patients' homes or facilities. The position emphasizes coordination with the care team, scheduling visits, and ensuring goals of care are met under physician orders.

Qualifications

• Current, active, and unrestricted RN or LPN licensure in the state of practice.
• A minimum of three (3) years of nursing experience.
• Palliative care, hospice, or home health care experience is preferred.
• CHPN (Certified Hospice and Palliative Nurse) is preferred.

Responsibilities

• Take and process Palliative Care referrals.
• Provide support to the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals between all providers.
• Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.
• Inform palliative team of all new admissions and discharges.
• Coordinate referrals utilizing the referral process.
• Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.
• Run billing report and address validation holds/billing errors.
