Job summary

PruittHealth is seeking a Hospice Palliative Care Bridge Nurse to join our team delivering compassionate care in patients' homes or care facilities in North Carolina. This role emphasizes coordinating goals of care, collaborating with the Palliative Care Nurse Practitioner and Medical Director, and ensuring timely documentation and billing accuracy.

The ideal candidate holds an active RN or LPN license in the state, with at least three years of nursing experience; palliative/hospice experience

Qualifications

• Current, active nursing license (RN or LPN) in state of practice.
• Minimum three years of nursing experience.
• Palliative care, hospice or home health experience preferred.
• CHPN certification preferred.

Responsibilities

• Take and process palliative care referrals.
• Support the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate care goals between all providers.
• Present the palliative program and obtain consents.
• Communicate assessment findings to attending physician.
• Complete documentation accurately and timely.
• Coordinate referrals and liaise with medical directors and nurse practitioners.
• Run billing reports and address validation holds.
