Job summary

Blue Ridge Care is seeking a compassionate LPN or RN for Extended Hours Hospice Care on a PRN basis. You will respond to urgent needs after-hours, perform nursing assessments, deliver direct hospice care, and coordinate with the interdisciplinary team to support comfort and dignity.

This role requires travel throughout the Blue Ridge Care territory and may include evenings, overnights, weekends, and holidays, with flexible scheduling and applicable shift differentials to support patient-focused

Qualifications

• Current Virginia RN or LPN license.
• Graduate of an accredited nursing program.
• Hospice, home health, or palliative care experience preferred.
• CPR certification required.
• Strong assessment and communication skills.

Responsibilities

• Respond to emergent calls during extended coverage hours.
• Provide urgent visits for symptom changes and end-of-life concerns.
• Deliver skilled nursing care per patient goals and orders.
• Assess patient conditions and communicate with the care team.
• Administer medications and treatments as prescribed.
• Support symptom management and patient comfort.
• Educate patients and families on disease progression and care techniques.
• Provide emotional support in challenging situations.
• Coordinate equipment, supplies, and services for home care.
• Collaborate with interdisciplinary team for continuity of care.
