Job summary

North Country Healthcare is seeking a Hospice Bereavement Coordinator & Volunteer Liaison to develop, coordinate, implement, and evaluate bereavement and volunteer programs across the hospice continuum.

The role collaborates with the Social Worker, IDT, and agency leadership, ensuring compliance with Medicare CoP and delivering emotional support and community education to patients, families, and caregivers. A bachelor's degree in a related field is required.

Qualifications

• Bachelor's degree in a field related to healthcare, social services, or administration is required or 8 years of related experience in lieu of formal education.
• Experience in hospice, palliative care, bereavement support, or closely related field preferred.
• Experience recruiting, coordinating, or supervising volunteers in healthcare or nonprofit settings preferred.

Responsibilities

• Coordinate and oversee the hospice bereavement program in accordance with Medicare Conditions of Participation.
• Provide pre-bereavement support and bereavement assessment during active hospice episodes.
• Coordinate volunteer recruitment, training, supervision, and utilization.
• Maintain documentation of bereavement contacts, risk assessments, and plans of care in EMR.
