Job summary

North Country Healthcare is seeking a Hospice Bereavement Coordinator & Volunteer Liaison to develop and oversee bereavement and volunteer programs. You will coordinate emotional support, grief resources, and community education, working with the hospice team to ensure Medicare CoP compliance and high-quality patient care.

Responsibilities include volunteer recruitment and supervision, interdisciplinary collaboration, and maintaining accurate documentation in the EMR while supporting families

Qualifications

• Bachelor's degree in Business Administration, Healthcare Management, Social Work, Psychology, Counseling, Human Services or related field or 8 extra years of experience in lieu of formal education.
• Proficiency with Microsoft Office; familiarity with EMR/documentation is preferred.
• Ability to maintain professional boundaries when working with bereaved individuals and families.

Responsibilities

• Develop, coordinate, implement, and evaluate hospice bereavement and volunteer programs.
• Provide emotional support and grief resources to patients, families, and community members.
• Recruit, coordinate, supervise, and retain hospice volunteers; ensure compliance with Medicare requirements.
