Job summary

North Country Healthcare in New Hampshire is seeking a Bereavement Coordinator & Volunteer Liaison to develop, coordinate, implement, and evaluate hospice bereavement and volunteer programs. You will work with the Social Worker, IDT, and leadership to ensure compliant, compassionate care.

The role emphasizes assessing bereavement needs, coordinating grief resources, and supervising volunteers, with a focus on quality and compliance under Medicare Conditions of Participation.

Qualifications

• Bachelor's degree in Business Administration, Healthcare Management, Social Work, Psychology, Counseling, Human Services, or related field is preferred.
• CT in Thanatology (CT) or GSP-C preferred; BLS/ACLS/PALS certification required or target within 2–3 years if not held.
• Valid Driver's license and reliable transportation required.

Responsibilities

• Coordinate and oversee the hospice bereavement program in accordance with Medicare Conditions of Participation.
• Provide pre-bereavement support and bereavement assessment during active hospice episodes.
• Maintain ongoing contact with bereaved family members for up to 13 months after a patient’s death.
• Lead volunteer recruitment, onboarding, training, and supervision to ensure compliant volunteer services.
• Participate in IDT meetings and QAPI activities; support data collection and reporting for quality monitoring.
