Job summary

North Country Healthcare in Littleton, NH is seeking a Bereavement Coordinator & Volunteer Liaison to develop, coordinate, and evaluate hospice bereavement and volunteer programs, ensuring compliance with Medicare Conditions of Participation and serving patients, families, and caregivers throughout the bereavement period.

Qualifications

  • Bachelor's degree required in a relevant field or 8 years of experience in lieu of formal education.
  • Certified in Thanatology (CT) preferred; GSP-C preferred.
  • BLS/ACLS/PALS or Heartsaver CPR AED certification required or obtained after hire.

Responsibilities

  • Provide bereavement services for at least 13 months after death.
  • Coordinate recruitment, training, supervision, and utilization of hospice volunteers.
  • Perform ongoing bereavement assessments and develop individualized plans of care.
  • Document all bereavement contacts and outcomes in the EMR.
  • Collaborate with the interdisciplinary team to support patients, families, and community education.
