Job summary

North Country Healthcare seeks a dedicated Bereavement Coordinator & Volunteer Liaison to develop, coordinate, implement, and evaluate hospice bereavement and volunteer programs. You will ensure Medicare CoP compliance while supporting patients, families, and caregivers throughout the hospice journey.

The role collaborates with the Social Worker, IDT, and agency leadership, providing emotional support, grief resources, and community education to enhance hospice care quality.

Qualifications

• Bachelor's degree in Business Administration, Healthcare Management, Social Work, Psychology, Counseling, Human Services, or a related field is required, or in lieu of formal education, an additional 8 years of experience.
• Certification in Thanatology (CT) or Grief Support Practitioner (GSP-C) preferred; BLS/ ACLS/PALS or Heartsaver CPR AED training may be required.
• Valid Driver's license and reliable personal transportation are required.
• Proficiency with Microsoft Office suite; familiarity with EMR documentation preferred.

Responsibilities

• Coordinate and oversee the hospice bereavement program in accordance with Medicare Conditions of Participation.
• Provide pre-bereavement support and bereavement assessment during active hospice episode.
• Lead recruiting, interviewing, onboarding, training, and retention of hospice volunteers.
• Participate in Interdisciplinary Team (IDT) meetings and QAPI activities.
• Ensure compliance with HIPAA and regulatory requirements; maintain documentation in the EMR.
