Job summary

North Country Healthcare (NCH) is seeking a Bereavement Coordinator & Volunteer Liaison in a full-time, hybrid role based in Littleton, NH. The position supports bereavement services and volunteer programs across the hospice continuum, aligning with Medicare requirements and a compassionate care model.

The role collaborates with the Social Worker and interdisciplinary teams to provide emotional support, grief resources, and community education while managing documentation and program outcomes

Qualifications

• Bachelor's degree in Business Administration, Healthcare Management, Social Work, Psychology, Counseling, Human Services, or related field is required or 8 years of experience in lieu.

Responsibilities

• Coordinate bereavement and volunteer programs.
• Provide emotional support and grief resources.
• Recruit, train, and supervise hospice volunteers.
• Collaborate with Social Worker and IDT for care planning.
• Ensure compliance with Medicare Conditions of Participation.
