Job summary

The Bereavement Coordinator at Covenant Living develops and manages a one-year bereavement program for families and caregivers of hospice patients, coordinating grief counseling, follow-up, and referrals for CovenantCare at Home. You will work with a multidisciplinary team to ensure timely services and community referrals.

You will provide individual and group counseling, support to hospice personnel, and contribute to quality improvement initiatives to enhance bereavement services.

Qualifications

• Bachelor’s degree or Theology degree or MSW with grief counseling qualifications.
• Experience with families/caregivers and patients through death/dying issues.
• Experience with clinical grief counseling.

Responsibilities

• Plan, implement, and maintain a bereavement program for families/caregivers for up to one year.
• Coordinate assessment and delivery of grief counseling (one-to-one, groups, follow-up).
• Provide bereavement information and referrals to community callers.
• Participate in hospice activities and quality improvement teams.
