Covenant Living and CovenantCare Home Health and Hospice seeks a Bereavement Coordinator to plan, implement, and maintain a bereavement program for families and caregivers for up to one year after a patient's death.

The role coordinates assessments and delivers grief counseling in one-to-one and group settings, while expanding community resource lists and providing information to callers. Collaboration with hospice teams and interns is expected.

Join us!The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from CovenantCare Hospice for up to one (1) year following the death of the individual hospice patient.Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.Participates in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups).Functions independently, but seeks and accepts guidance from other members of the interdisciplinary group, or from member of the community.Actively participates in quality assessment performance improvement teams and activities.EDUCATION AND WORK EXPERIENCE:Bachelor’s degree or equivalent theological degree from college or university OR Master's degree in Social Work with qualifications of experienced clinical grief counseling.Experience:Experience with families/caregivers and patients through death/dying issuesExperience with clinical grief counseling.KNOWLEDGE, SKILLS AND ABILITY:Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.Ability to work as a team member.Knowledge and commitment to hospice philosophy.Demonstrates good verbal and written communication, and organization skills.PHYSICAL REQUIREMENTS:This position requires mobility to move around from one area to another. The majority of time is spent sitting, walking, bending, stooping and driving. Employee must have the ability to lift loads not normally exceeding medium weight (35 pounds). Manual dexterity, keen vision and hearing accuracy are necessary in driving a vehicle.Compensation Pay Range:$23.62 - $32.16 per hourReasonable Pay EstimateA reasonable estimate of the pay range for this position is $23.62 - $32.16 per hour.
