Job summary

Elara Caring seeks a Hospice Clinical Liaison to lead patient referrals, coordinate transitions home and collaborate with hospital and field teams. You’ll educate families about hospice options, verify insurance, and support care planning from hospital to home.

You will operate across referral networks, participate in care coordination, and remain current on GIP guidelines to ensure compliant, high-quality hospice services.

Qualifications

  • Bachelor of Science degree in Biology, Life Science, Anatomy, Physiology, or a related field is preferred.
  • Three (3) years’ practical experience and experience in case management in a clinical setting is preferred.
  • RN License required.
  • Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
  • TRAVEL: As needed, based on territory demographic or organizational need

Responsibilities

  • Identifies, collects, and reviews referral information to support patient eligibility and care coordination.
  • Conducts joint customer visits and relationship-building activities with the Area Vice President of Sales, Account Executives, and key referral sources, including execution of Quarterly Business Review (QBR) plans.
  • Collaborates with referral sources, healthcare providers, and hospital staff to facilitate referrals and coordinate patient care.
  • Educates patients and families on available services and care options.
  • Partners with hospital staff, physicians, case managers, and other healthcare professionals to support discharge planning and ensure patients receive the appropriate level of care.
  • Coordinates transitions of care from healthcare facilities to the home environment, ensuring a seamless patient experience.
  • Collaborates with Intake and clinical teams to verify insurance eligibility, coordinate services, and support continuity of care.
  • Serves as a liaison between hospital staff and operational teams to enhance communication and patient outcomes.
  • Maintains ongoing communication with referral sources and healthcare partners regarding available services and patient care coordination.
  • Develops and maintains positive relationships within the community and referral network.
  • Completes the initial clinical evaluation to assess Local Coverage Determination (LCD) criteria and communicate findings to the certifying provider(s), including supporting clinical documentation to substantiate hospice eligibility.
  • Maintains expert-level knowledge of General Inpatient (GIP) hospice regulations, standards of care, documentation requirements, and reimbursement guidelines to promote compliant, high-quality patient care and appropriate utilization of the GIP level of care.
  • Educates patients, families, facility partners, and referral sources regarding hospice philosophy, eligibility criteria, and levels of care, ensuring understanding of the interdisciplinary plan of care and available hospice services.
  • Provides education and clinical consultation to hospital and facility staff regarding hospice philosophy, symptom management, GIP criteria, levels of care, and best practices for end-of-life care.
  • Reviews and collaborates on medication management, physician orders, infection prevention considerations, and care coordination activities to support safe, effective transitions into hospice services and the provision of GIP care when indicated.
  • Serves as the clinical subject matter expert during the referral and admission process for GIP, providing guidance to referral sources, providers, patients, and families regarding hospice eligibility, regulatory requirements, levels of care, and appropriate care planning.

Education

As a Hospice Clinical Liaison, you’ll contribute to our success in the following ways:
