Job summary

UnitedHealth Group is seeking a Bereavement Coordinator to supervise bereavement services aligned with patient and family needs. You will deliver compassionate care and document progress using Visit Notes, Care Plans and EGSS scores as part of a holistic support team.

This role emphasizes active listening, coordination of grief support, and creating a Plan of Care that covers services and frequency up to 13 months post-death. Local travel may be required.

Qualifications

• Experience in grief and loss counseling.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Establishes a Plan of Care addressing bereavement needs, detailing services and frequency up to 13 months post-patient death
• Demonstrates active listening and provides impactful suggestions during phone and home visits within LHC guidelines
• Leads and coordinates at least 12 Grief Support Group sessions per year
• Maintains Bereavement Binder with information and attendance sheets from various events and presentations
• Provides individualized and compassionate care for each bereaved, documented in Visit Notes, Bereavement Risk Assessment, Care Plan, and EGSS scores
