Job summary

Angels Care Hospice is seeking a Bereavement Coordinator to plan, implement, and maintain a comprehensive bereavement program for families and caregivers. The role coordinates grief counseling, supports hospice staff, and helps expand and refine services through annual evaluations.

The candidate will assess care needs, participate in quality improvement teams, and implement care plans following a patient’s death, ensuring ongoing updates and compliance with hospice philosophy.

Qualifications

• Bachelor's degree or equivalent theological degree from college or university.
• Master's degree in Social Work with qualifications of experienced clinical grief counseling.
• Experience with families/caregivers and patients through death & dying issues.
• Ability to work as a team member.
• Knowledge and commitment to hospice philosophy.
• Good verbal and written communication, and organization skills.
• Understanding of grief response and experience with counseling.
• Licensed driver with an automobile that is insured.

Responsibilities

• Plan, implement, and maintain a bereavement program to meet the needs of families & caregivers.
• Coordinate assessment and delivery of grief counseling services.
• Provide bereavement support to hospice personnel coping with work-related grief.
• Evaluate and plan services to meet personnel support needs.
• Participate in quality improvement and hospice activities.
• Determine level of care and support for bereaved families following patient death.
• Update and revise the bereavement care plan as needed.
• Implements Plan of Care provisions related to bereavement needs.
