Job summary

Ovation Hospice of Salt Lake seeks a compassionate Hospice Aide Scheduler to coordinate daily and weekly schedules for Hospice Aides/CNAs, ensuring timely, consistent care aligned with each patient’s care plan and staff availability. The role requires strong organizational skills, clear communication with patients, families and clinical staff, and the ability to adapt when schedules change.

Qualifications

• Highly organized with strong attention to detail.
• Excellent written and verbal communication with patients, families, and staff.
• Able to multitask and prioritize in a fast-paced environment.
• Capable of working independently and as part of a team.
• Experience in hospice, home health scheduling, or staffing is preferred.

Responsibilities

• Coordinate and maintain daily/weekly schedules for Hospice Aides/CNAs based on patient needs.
• Ensure visits occur consistently and align with care plans.
• Match aides to patients considering location, availability, and skills.
• Communicate schedule changes to aides, nurses, patients, and families.
• Monitor missed or rescheduled visits and address coverage needs quickly.
• Assist with filling open shifts and coverage when aides are unavailable.
• Maintain scheduling records and documentation accurately.
• Review upcoming schedules to identify staffing gaps.
• Work with clinical team to adapt schedules to patient needs.
• Ensure schedules align with organizational policies and care plans.
• Respond professionally to questions from patients and staff.
• Assist onboarding and scheduling of new Hospice Aides.
• Maintain patient information confidentiality.
• Support backlog tasks and other admin/scheduling duties as needed.
