Job summary

Choice Health at Home seeks a compassionate Hospice Aide for PRN home-based care in Waxahachie, TX. You will support patients with personal care in their homes, assist with transfers, and help maintain a safe, clean environment under RN supervision.

You will report health changes, document care, and communicate respectfully with patients, families, and staff to sustain the high quality of hospice services. This role emphasizes empathy and teamwork.

Qualifications

• Experience in direct patient care in home or hospice settings preferred.
• Ability to assist with transfers and ambulation safely.
• Familiarity with infection control principles and safe home environments.

Responsibilities

• Provide direct patient care to patients under the RN and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care rendered to ensure continuity of care.
• Maintain clean, safe, and comfortable home environment.
• Promote respectful communication with patient/family and staff.
• Respect patient privacy and property at all times.
• Assist with proper nutrition and fluid intake.
• Support efficient operation and time management of the agency.
• Perform treatments and procedures as directed by policy.
• Align with agency philosophy and administrative policies to ensure quality care.
