Job summary
Peoples Hospice and Palliative Care is seeking a Hospice Aide to provide direct patient care in patients' homes under the supervision of a Registered Nurse. You will implement nursing measures, assist with activities of daily living, and support caregiving staff to maintain comfort and dignity for patients.
The role requires compassionate care, ability to work independently, and adherence to home health policies.
Qualifications
- At least one year experience in hospice or home health care.
- Holds appropriate certificate and/or has completed required curriculum; competency evaluated per regulations. LPNs with CNA certification considered.
Responsibilities
- Participates in patient care conferences and meetings.
- Attends continuing education as approved by the agency.
- Treats patients, physicians and colleagues with care, courtesy and respect.
- Provides personal hygiene and care as directed by the RN.
- Assists patients with bathroom needs and transfers.
- Assists with mobility and ambulation as applicable.
- Assists with prescribed exercises and rehabilitation activities as instructed.
- Measures and records vital signs and intake/output.
- Completes daily care work sheets and visits reports.
- Maintains professional appearance and complies with uniform policy.
- Notifies supervisor of changes in patient condition.
- Observes infection control and safety policies.
