Job summary

Guardian Angel Hospice is seeking a dedicated Hospice Aide to provide compassionate direct patient care in patients' homes and healthcare facilities under the supervision of the RN Case Manager. You will assist with bathing, grooming, oral hygiene, skin care, and other personal needs, while promoting dignity and safety during ADLs.

You will report observations to the RN Case Manager and team, maintain professional boundaries, assist with transfers using hoyer lifts and mobility aids, and offer

• Current CNA or HHA certification in the state of Indiana.
• Reliable transportation and a valid driver’s license.
• Flexibility to participate in the on-call rotation schedule.
• Physical ability to perform the duties of the role.
• Strong time management skills and the ability to work independently while being part of a supportive team.

Responsibilities

• Provide direct patient care under the supervision of the RN Case Manager, including assistance with bathing, grooming, oral hygiene, skin care, nail care, and other personal needs.
• Support patients with activities of daily living (ADLs) in a way that promotes dignity, comfort, and safety.
• Report observations and changes in patients' physical or emotional condition to the RN Case Manager and interdisciplinary team.
• Respect professional boundaries while providing care in homes and healthcare facilities.
• Safely assist with transfers and mobility, including the use of hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches.
• Offer emotional support and compassionate presence to patients and caregivers.
