Job summary

Guardian Angel Hospice in Avon, Indiana is seeking a dedicated Hospice Aide to join our caring team. You will provide direct patient care and assist with activities of daily living in the patient’s home or care setting, guided by an individualized Plan of Care.

Responsibilities include supporting comfort and dignity, reporting changes to the RN Case Manager, and safely assisting with transfers. Strong communication and compassionate presence are essential for this role.

Qualifications

• Current CNA or HHA certification in Indiana.
• Reliable transportation and valid driver's license.
• Flexibility to participate in on-call rotation.

Responsibilities

• Provide direct patient care under the supervision of the RN Case Manager, including bathing, grooming, oral hygiene, skin care, nail care, and other personal needs.
• Support patients with activities of daily living (ADLs) in a way that promotes dignity, comfort, and safety.
• Report observations and changes in patients’ physical or emotional condition to the RN Case Manager and interdisciplinary team.
• Respect professional boundaries while providing care in homes and healthcare facilities.
• Safely assist with transfers and mobility, including the use of hoyer lifts, gait belts, wheelchairs, walkers, canes, and crutches.
• Offer emotional support and compassionate presence to patients and caregivers.
