Job summary

Heart to Heart Hospice is seeking a Hospice Aide (CNA) to deliver direct patient care under the RN, following the Plan of Care. This full-time role supports patients with life-limiting illnesses in the community.

You will assist with transfers, document care, maintain infection control, and communicate with families and staff. A valid CNA license, HS diploma or GED, and at least one year of relevant experience are required; driving and insurance are needed.

Qualifications

• High School Diploma or GED required (preferred).
• Unencumbered CNA license currently.
• At least 1 year experience as Hospice Aide or Nursing Assistant.
• Reliable transportation with valid auto liability insurance.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist patient with transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
• Maintain infection control practices.
• Communicate respectfully with patient, family, and staff.
• Submit documentation timely.
• Perform other duties as required.
• Comply with laws, Medicare regulations and agency policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
